1. Overview

The My Smile Story Sweepstakes (the “Sweepstakes”) is a promotional sweepstakes sponsored by Harvey & Thomas Orthodontics, LLC, an Alabama limited liability company, located at 58 Mobile Street North, Mobile, Alabama 36607 (“Sponsor”), to celebrate smile stories from the Alabama, Florida and Mississippi communities and award one eligible entrant a Disney vacation prize, subject to these Official Rules. No purchase, payment, treatment, consultation, orthodontic services, social media post, testimonial, media consent, or HIPAA authorization is required to enter or win. A purchase or payment will not increase the chances of winning.

These Official Rules govern the Sweepstakes and are intended to be read together with the registration form, any winner documentation, and Sponsor’s privacy notice available at https://harveyandthomasortho.com/privacy-policy/. In the event of a conflict between promotional copy and these Official Rules, these Official Rules control.

2. Sweepstakes Period

The Sweepstakes begins at 12:00 AM Central Time on August 17, 2026 and ends at 11:59 PM Central Time on November 16, 2026 (the “Sweepstakes Period”).

3. Eligibility

The Sweepstakes is open only to legal residents of Alabama, Florida and Mississippi who are eighteen (18) years of age or older at the time of entry and that comply with these Official Rules. The Sweepstakes is not limited to current or former patients of Sponsor and is not conditioned on becoming a patient. An adult parent or legal guardian may submit an optional story or related content concerning the adult’s minor child, but the adult parent or legal guardian is the entrant, is solely responsible for the submission, represents that he or she has authority to act for the minor in connection with the content submitted, and the minor is not an entrant.

The following persons are not eligible to enter or win: (a) Sponsor; (b) any travel agency, marketing agency, advertising agency, social media agency, printer, technology vendor, prize supplier, or other vendor involved in developing, administering, promoting, fulfilling, or evaluating the Sweepstakes; (c) any persons involved in the selection of the winner or verification of entries; (d) immediate family members (defined as spouse, parent, child, sibling, grandparent, grandchild, and any of the foregoing by marriage, regardless of where they reside) of any of the foregoing; and (e) persons living in the same household (whether related or not) as any of the foregoing.

The Sweepstakes is void outside Alabama, Florida and Mississippi and where prohibited or restricted by law.

4. Entry Submission Certification; Agreement to Official Rules

By signing or electronically submitting the registration form, each entrant makes the following single entry-submission certification: the entrant (a) agrees to be bound by these Official Rules and by the decisions of Sponsor, which are final and binding in all matters relating to the Sweepstakes; (b) represents and warrants that the entrant is eligible and that any information submitted is accurate and complete; (c) represents and warrants that any Content submitted in connection with the Sweepstakes complies with these Official Rules; and (d) agrees that Sponsor may verify eligibility, identity, and compliance at any time. Winning a prize is contingent upon fulfilling all requirements of these Official Rules.

5. How to Enter

5.1. Free Base Entry

To receive one (1) base entry, an eligible entrant must, during the Sweepstakes Period:

  1. Visit https://harveyandthomasortho.com/smilestory/;
  2. Complete the registration form with all required information, including the entrant’s state of legal residence and the single entry-submission certification described in Section 4; and
  3. Submit the form during the Sweepstakes Period.

The registration form is the only required entry step. Submission of the registration form, including the single entry-submission certification described in Section 4, constitutes one (1) base entry into the Sweepstakes. Only fully completed registration forms received during the Sweepstakes Period are eligible. No purchase, treatment, consultation, office visit, social media activity, testimonial, media release, publicity consent, or HIPAA authorization is required for the base entry.

5.2. Maximum Bonus Entries

An eligible entrant may earn up to three (3) bonus entries total, and no more, during the Sweepstakes Period by completing one of the bonus entry methods in Section 5.3 or 5.4 below. Accordingly, the maximum total number of entries per entrant is: one (1) base entry, plus up to three (3) bonus entries, for a maximum of four (4) total entries per entrant for the entire Sweepstakes Period. An entrant may not stack multiple bonus methods to exceed three (3) bonus entries total.

5.3. Optional Social Bonus Entry Method

After submitting the base entry form, an entrant may earn three (3) bonus entries by completing one of the following optional social media methods during the Sweepstakes Period:

Important conditions for the optional social bonus method:

  • The social post or Facebook comment must be made after the entrant submits the base registration form.
  • The post or comment must remain visible and verifiable through the drawing date unless Sponsor instructs otherwise.
  • The account used must match, or be reasonably traceable to, the handle or identity supplied on the registration form.
  • A Facebook personal feed post is welcome but does not itself earn bonus entries unless Sponsor expressly states otherwise in these Official Rules.
  • Instagram Stories, disappearing posts, and private posts do not qualify for bonus entries unless Sponsor expressly designates a verification process for them; Sponsor has no obligation to chase or recover inaccessible Content.
  • Sponsor is not responsible for platform outages, privacy settings, deleted posts, hidden tags, shadow bans, account restrictions, or inability to identify a post.

5.4. Alternative Non-Social Bonus Entry Method

To ensure that social posting is not required, an eligible entrant may instead earn the same three (3) bonus entries without using social media by submitting an alternative bonus entry as follows:

During the Sweepstakes Period, after submitting the base registration form, send Sponsor a request for the alternative non-social bonus entries through the email address, form, or other submission method designated by Sponsor for that purpose. The request must include: (a) entrant’s full legal name; (b) email address and mobile number used on the registration form; and (c) a clear request for the alternative non-social bonus entries. If the submission complies with these Official Rules, the entrant will receive three (3) bonus entries.

This alternative method is intended to provide entry value equal to the optional social bonus method. No public posting, tag, or social account is required for the alternative non-social bonus method.

6. Required Disclosure for Incentivized Social Posts

Because optional social posts may earn bonus entries, each qualifying social post must clearly disclose that it is incentivized by using the hashtag #HarveyandThomasSweepstakes. Any advertising, landing page, social post, or other promotional copy must not state or imply that the Sweepstakes is open outside Alabama, Florida, and Mississippi, that any purchase, treatment, social post, media release, HIPAA authorization, or marketing consent is required, or that any prize, guaranteed reward, gift card, entrant gift, or runner-up prize is available unless expressly included in these Official Rules.

7. Entry Limits

Unless Sponsor expressly approves otherwise in writing, a limit of one (1) base entry per person, e-mail address, and mobile number will be permitted during the Sweepstakes Period. A limit of three (3) total bonus entries per entrant during the Sweepstakes Period. If multiple entries are received from the same person, email address, mobile number, or account in excess of the stated limits, Sponsor may disqualify the excess entries or all associated entries in Sponsor’s discretion.

8. Content Requirements

Any story, post, comment, image, video, audio, caption, or other material submitted, posted, transmitted, or otherwise provided in connection with the Sweepstakes (collectively, “Content”) must comply with these requirements.

As part of the single entry-submission certification in these Official Rules, each entrant represents and warrants that the entrant’s Content: (a) is truthful to the entrant’s knowledge; (b) is original to the entrant or used with all permissions necessary for the uses contemplated here; (c) does not infringe, misappropriate, or violate any copyright, trademark, privacy, publicity, or other rights of any person or entity; (d) does not contain any person other than the entrant or entrant’s family members unless the entrant has obtained all required permissions; (e) does not include a minor unless the entrant is the minor’s parent or legal guardian or has obtained legally sufficient consent from the minor’s parent or legal guardian; (f) does not disclose another person’s medical, health, or confidential information without permission; (g) does not contain false, deceptive, misleading, defamatory, disparaging, obscene, sexually explicit, hateful, threatening, harassing, unlawful, violent, dangerous, or discriminatory material; (h) does not promote illegal activity, unsafe activity, or misuse of orthodontic or dental appliances; (i) does not contain viruses, malware, corrupted files, or harmful code; and (j) complies with applicable platform terms and community guidelines. By submitting Content, each entrant grants Sponsor a non-exclusive, royalty-free license to use, reproduce, display, and retain such Content solely as reasonably necessary for Sweepstakes administration, entry and bonus-entry verification, fraud prevention, winner selection and notification, prize fulfillment, and legally required recordkeeping. Advertising, publicity, testimonial, reposting, resharing, or other marketing use of Content requires a separate media release, publicity consent, HIPAA authorization, or other consent where required by law.

Sponsor may disqualify any entrant or remove or disregard any Content that Sponsor determines, in its sole but reasonable discretion, violates these Official Rules, is inconsistent with Sponsor’s reputation, creates legal or regulatory risk, or is otherwise unsuitable for publication or Sweepstakes administration.

9. Entry Verification and Proof of Submission

Proof of sending or posting is not proof of receipt, eligibility, or compliance. Without limitation: (a) a screenshot does not by itself prove valid entry; (b) an auto-generated confirmation does not waive verification requirements; (c) Sponsor may require proof of identity, age, residency, authority, and compliance before counting any entry or awarding any prize. Sponsor may request additional documentation, including government-issued identification, proof of legal residency in Alabama, Florida, or Mississippi; proof that the entrant controls the social media account used, documentation confirming parental or guardianship authority for any minor featured in Content, third-party releases for identifiable persons appearing in Content; and such other documents as Sponsor reasonably deems necessary. Failure to provide requested verification in the time stated by Sponsor may result in disqualification.

10. Automated Entries; Fraud; Technical Issues

Entries generated by script, macro, bot, artificial means, vote exchange, bulk account creation, or any automated, deceptive, or abusive means are void. Sponsor may disqualify any individual who violates these Official Rules, provides false information, or acts in an unsportsmanlike, disruptive, abusive, or fraudulent manner. Sponsor is not responsible for lost, late, incomplete, invalid, unintelligible, corrupted, misdirected, delayed, or undelivered entries; platform, website, network, server, carrier, internet, hardware, software, browser, or mobile device failures; human error; failed, incomplete, or unavailable social tags, comments, mentions, or posts; inaccessible privacy settings or deleted Content; or any other errors or malfunctions that may affect participation or administration. If the Sweepstakes cannot be administered as planned due to infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, platform restrictions, legal developments, public health events, natural disasters, or any other causes that corrupt or affect the administration, security, fairness, integrity, or proper conduct of the Sweepstakes, Sponsor may, in its sole discretion and to the extent permitted by law, suspend, modify, or terminate the Sweepstakes and, if terminated, select the winner in a fair manner from among all eligible, non-suspect entries received before termination.

11. Random Drawing; Odds

On or about November 17, 2026, Sponsor (or a third-party administrator designated by Sponsor, if any) will conduct a random drawing from among all eligible entries received during the Sweepstakes Period, including valid base entries and valid bonus entries. The drawing will determine one (1) potential winner. Odds of winning depend on the total number of eligible entries received. No aspect of the winner selection process will be based on the quality, emotional impact, popularity, creativity, length, or commercial usefulness of any story or Content.

12. Winner Notification

The potential winner will be notified by email, phone, or text message using the contact information provided on the registration form, within a commercially reasonable time after the drawing; Sponsor is not responsible for spam filters, blocked calls, undeliverable messages, or other delivery failures. To remain eligible, the potential winner must respond within five (5) business days after Sponsor’s first attempted notification. If a potential winner cannot be contacted; does not respond in time; refuses the prize; is found to be ineligible; fails to provide required documentation; fails to comply with these Official Rules; or cannot accept the prize within the stated travel conditions, then Sponsor may disqualify that potential winner and select an alternate potential winner in a subsequent random drawing from the remaining eligible entries, time permitting.

Sponsor may repeat this process until the prize is awarded or Sponsor determines, in its discretion, that the prize cannot reasonably be awarded.

13. Winner Documentation

As a condition of receiving the prize, the potential winner may be required to execute and return, within the time specified by Sponsor, an affidavit or declaration of eligibility; a liability/publicity release, where lawful; an IRS Form W-9; any tax documentation reasonably required by Sponsor; any travel-related releases, acknowledgments, or guest documentation; and any additional documents reasonably required for verification and prize fulfillment.

If the winner proposes guest travelers, Sponsor may require each adult guest to sign a liability/publicity release and may require the parent or legal guardian of each minor guest to sign a release and all travel documents on the minor’s behalf.

Neither publicity consent nor HIPAA authorization is a condition of entry. However, if the prize or later promotional use would involve the use of the winner’s name, likeness, testimonial, treatment information, or protected health information beyond what is permitted by law, Sponsor may request separate written authorizations, and any such authorizations must be separately presented and separately consented to where required.

14. Prize

There will be one (1) prize awarded, consisting of a Disney vacation package for up to four (4) persons total (the winner and up to three (3) guests), with a maximum approximate retail value (“ARV”) of Five Thousand Dollars (US $5,000.00). The prize will be fulfilled as a travel agency-booked package and may include, subject to final booking and availability, up to four (4) nights of hotel accommodations, up to three (3) days of theme park admission for up to four (4) persons total; and/or other travel components expressly described by Sponsor in winner materials. All specific components, providers, travel dates, inventory, routing, and booking details are subject to availability and Sponsor’s final approval. The total maximum ARV of all prizes and prize-related items offered or awarded in connection with the Sweepstakes will not exceed Five Thousand Dollars (US $5,000.00). No additional prizes, runner-up prizes, guaranteed rewards, gift cards, entrant gifts, or other prize-related items may be offered or awarded unless separately reviewed and expressly included within the total prize value stated in these Official Rules.

Unless Sponsor expressly states otherwise in writing and includes the value within the total maximum ARV stated above, the prize does not include airfare, transportation to and from the destination, baggage fees, seat selection fees, upgrades, travel insurance, resort fees, parking, meals not expressly included, souvenirs, spending money, incidental expenses, gratuities, childcare, pet care, wages, salary, time off work, gift cards, additional prizes, any taxes; or any other costs not specifically stated as included. The winner and guests are solely responsible for all unspecified costs and expenses.

Travel must be completed within twelve (12) months of the prize being awarded (or such longer period as Sponsor may allow in its sole discretion), subject to blackout dates, seasonal pricing, destination capacity, theme park reservation systems, operational changes, and provider availability.

Travel dates are subject to Sponsor approval and supplier availability. Certain holidays, peak seasons, school breaks, and special events may be unavailable. The actual retail value may vary depending on booking date, season, and selected components. If the actual value of the prize is less than the stated maximum ARV, the winner will not receive the difference in cash, credit, or any substitute. If the winner does not use any portion of the prize, that portion will be forfeited without compensation. Once booked, travel arrangements may be nonrefundable and nontransferable. Missed flights, missed reservations, no-shows, or failure to travel during the permitted window may result in total forfeiture. Sponsor is not responsible for supplier cancellations, schedule changes, overbooking, weather delays, acts of God, public health restrictions, labor disputes, or other events outside Sponsor’s control.

The prize is for up to four (4) persons total, including the winner. All guest arrangements are subject to these Official Rules and Sponsor approval. Any guest who is a minor must travel with the winner or another adult authorized by Sponsor and by the minor’s parent or legal guardian. The winner is responsible for the conduct of all guests. Each traveler must possess and present any required identification, tickets, confirmations, or travel documents. Sponsor is not responsible if any traveler is denied boarding, denied entry, refused admission, or otherwise unable to travel or participate due to lack of documentation, legal restrictions, illness, behavior, or failure to meet supplier requirements.

The prize is nontransferable, nonassignable, and not redeemable for cash except at Sponsor’s sole discretion. No substitution by the winner is permitted. Sponsor reserves the right to substitute the prize, or any portion of it, with a prize of equal or lesser value if the advertised prize, or any portion of it, becomes unavailable for any reason, provided that the total prize value may not exceed the maximum ARV stated in these Official Rules.

The winner is solely responsible for all federal, state, and local taxes arising from acceptance or use of the prize, including any taxes applicable in the winner’s state of residence. Sponsor will require the winner to provide a completed IRS Form W-9 and may issue an IRS Form 1099-MISC or other tax reporting form. The winner should consult the winner’s own tax advisor regarding the tax consequences of accepting or using the prize.

15. No Patient Status Requirement; Separate Consents

Entry into the Sweepstakes is not conditioned on being a current, former, or prospective patient; scheduling or completing treatment; purchasing any product or service; posting on social media; granting publicity consent; granting a testimonial or media release; or signing a HIPAA authorization. Any media release, testimonial consent, publicity consent, or HIPAA authorization used by Sponsor must be presented separately where required by law, must not be bundled into the single entry-submission certification, and is not part of the mandatory conditions for the base entry. If an entrant voluntarily provides a separate authorization permitting Sponsor to use the entrant’s story, image, likeness, treatment information, or protected health information for marketing or publicity, that authorization will be governed by its own terms.

16. Publicity

Except where prohibited by law, and except to the extent a separate authorization is required by law, acceptance of the prize constitutes the winner’s consent for Sponsor and its designees to use the winner’s name, city, likeness, biographical information, voice, statements about the Sweepstakes, and prize information for advertising, publicity, and promotional purposes in any media now known or later developed, without additional compensation, notice, review, or approval. Notwithstanding the foregoing, Sponsor will not use or disclose protected health information except as permitted by law or pursuant to a separate valid authorization where required, will not state or imply that the winner is or was a patient of Sponsor unless separately authorized, and if Sponsor wishes to use treatment photos, treatment results, or other protected health information in marketing, Sponsor will obtain separate consent where legally required.

17. Privacy

Information collected in connection with the Sweepstakes will be used by Sponsor (and any third-party administrator designated by Sponsor, if any) for purposes of administering the Sweepstakes, verifying eligibility, contacting entrants, fulfilling the prize, complying with legal, accounting, tax, and recordkeeping obligations; and any other uses disclosed in Sponsor’s privacy notice. If an entrant separately opts into marketing communications, that entrant’s information may also be used in accordance with the terms of that consent. Marketing consent must be presented as a separate optional choice and is not required to enter.

18. Release of Meta; Platform Non-Sponsorship

The Sweepstakes is in no way sponsored, endorsed, administered by, or associated with Meta Platforms, Inc., Facebook, or Instagram. By participating, the entrant releases Meta Platforms, Inc., Facebook, and Instagram from any liability arising out of or related to the Sweepstakes to the fullest extent permitted by law. Any questions, comments, or complaints regarding the Sweepstakes must be directed to Sponsor, not to Meta Platforms, Inc., Facebook, or Instagram.

19. Walt Disney Company Non-Affiliation

This Sweepstakes is NOT sponsored, endorsed, administered by, or affiliated with The Walt Disney Company, Disney Destinations, LLC, Disney Parks, Experiences and Products, or any of their respective parents, subsidiaries, or affiliates. Any reference to “Disney” is solely to describe the intended destination or nature of the prize. All Disney names, marks, and properties are the property of their respective owners.

20. Release; Assumption of Risk; Limitation of Liability

As part of participation in the Sweepstakes and, for the winner, acceptance of the prize, each entrant and the winner, for himself or herself and on behalf of his or her heirs, executors, administrators, representatives, and assigns, to the fullest extent permitted by law hereby releases and holds harmless the Sponsor and each of its affiliates, subsidiaries, officers, directors, members, managers, employees, representatives, agents, successors, assigns, and each of their respective third-party administrators, travel agencies, prize suppliers, and other vendors involved in developing, administering, promoting, or fulfilling the Sweepstakes (collectively, the “Released Parties”) from and against any claim, demand, action, damage, loss, cost, or liability arising out of or relating to participation in the Sweepstakes, submission or use of any Content, acceptance, possession, use, misuse, or non-use of the prize, travel or participation in prize-related activities; or any technical or administrative errors in connection with the Sweepstakes, and hereby assumes all risks of travel, attendance, and participation in prize-related activities. Each entrant further agrees to indemnify and hold harmless the Released Parties from and against any third-party claim, demand, loss, damage, cost, or expense (including reasonable attorneys’ fees) arising out of or relating to any Content submitted by such entrant or any breach of these Official Rules by such entrant.

Nothing in these Official Rules excludes liability to the extent such liability cannot be excluded under applicable law.

To the fullest extent permitted by law, the Released Parties are not responsible for indirect, incidental, consequential, or punitive damages, and any recovery shall be limited to actual out-of-pocket costs associated with entering the Sweepstakes, if any.

21. Force Majeure; Modification; Cancellation

Sponsor reserves the right, in its sole discretion, to modify, suspend, or terminate the Sweepstakes or prize fulfillment, in whole or in part, if Sponsor determines that the Sweepstakes cannot be conducted or the prize cannot be awarded as planned due to causes beyond Sponsor’s reasonable control, including natural disasters; fire; flood; severe weather; epidemic or pandemic; public health restrictions; acts of government; war; terrorism; civil disturbance; labor disputes; transportation interruption; technology failures; or legal or regulatory changes. Any modification of prize fulfillment must remain subject to the total maximum ARV and no-additional-prize limitations stated in Section 14. If the Sweepstakes is terminated early, Sponsor may, where feasible and lawful, select the winner from eligible entries received before termination.

22. Disputes; Governing Law; Venue

All disputes, claims, and causes of action arising out of or related to the Sweepstakes or any prize shall be resolved individually, without resort to any form of class action. The Sweepstakes and these Official Rules shall be governed by the laws of the State of Alabama, without regard to conflict-of-law principles, subject to any non-waivable rights an entrant may have under applicable law in the entrant’s state of legal residence. Exclusive jurisdiction and venue for any action arising out of or relating to the Sweepstakes shall lie in the state courts and, if applicable, federal courts located in Mobile County, Alabama, and the entrant consents to such jurisdiction and venue to the fullest extent permitted by law.

23. Severability; Waiver

If any provision of these Official Rules is determined to be invalid, illegal, or unenforceable, the remaining provisions shall remain in full force and effect to the fullest extent permitted by law. Sponsor’s failure to enforce any provision of these Official Rules shall not constitute a waiver of that provision or any other provision.

24. Sponsor Reservation of Rights

Sponsor reserves the right to interpret these Official Rules and resolve all questions or disputes arising under them in a manner consistent with the promotional purpose of the Sweepstakes and applicable law. Sponsor’s decisions are final and binding in all matters relating to the Sweepstakes.

These Official Rules will be made available on Sponsor’s designated Sweepstakes landing page. For the name of the winner, available twenty (20) days after the drawing date, send a self-addressed stamped envelope to: Harvey & Thomas Orthodontics, LLC, 58 Mobile Street North, Mobile, Alabama 36607, Attn: My Smile Story Sweepstakes Winner, within sixty (60) days after the drawing date.
